Consultation Process

Initial Consultation

An initial consultation allows time to review the situation, understand the systems involved, and determine whether Rehabtree’s services are appropriate for the circumstances.

Case Review and Strategy Development

If services move forward, available documentation may be reviewed to identify care gaps, system barriers, and potential strategies to support access to necessary services and coordinated care.

Ongoing Navigation and Advocacy

Depending on the situation, services may include documentation analysis, care coordination support, appeals strategy development, or guidance navigating complex healthcare and disability systems.

Dr. DiGiovanni was the angel I needed when my elderly mom’s medical needs reached the point where she became unable to care for herself and live in her own home. She met with me and helped me navigate how to get my mom top notch medical coverage, and consulted with her associates to find my mom an assisted living facility where she was both medically cared for and involved in activities that stimulated her mind and body. She also provided me with professional contacts for PT/OT outside of what the facility’s standard was. When my mom had a medical crisis, Dr. DiGiovanni showed up late at night to help troubleshoot the underlying cause and suggested how medical intervention could better the situation. She had many doctor-to-doctor conversations with the facility doctors, as well as hospital and rehab personnel, to assist in my mom’s care and ensure that my mom’s situation was understood and her needs were met. When my mom’s illness progressed to end-stage, Dr. DiGiovanni supported me in transitioning my mother to both skilled nursing and palliative care by providing the providers with my mom’s medical history as well as an extensive history of my mom’s needs. She also personally checked in on my mom throughout the last four years of my mom’s illness. She always pushed forward, always offered solutions, and always offered compassion and positivity. I could not have given my 92-year-old mother the best life possible without Dr. DiGiovanni’s advocacy.
